ADMISSIONS REGISTERED NURSE, FT

Location: Brevard County Under the general direction of the Director of Business Development, the Admission Registered Nurse is responsible for determining the eligibility and preparation of patient/ family need and the initial written plan of care. Position works closely with the Intake Coordinator, Clinical Service Department, and the Director of Business Development to ensure timely processing of all referrals. Position serves as a resource for questions/issues from the health care community and HOSF staff. Position is full time and will include working weekends
Requirements: Requires an Associate degree in nursing. Bachelor’s or Certified Hospice and Palliative Care Nurse preferred. Requires a valid current Florida Registered Nurse License and a valid Florida Driver’s License with reliable transportation. Three years hospice experience preferred. Previous sales experience preferred. Ability to work flexible hours. Must understand the dynamics of human behavior and ability to communicate with patients and families in a crisis. Ability to communicate effectively with referral sources, and colleagues. Excellent oral and written communication, clinical and administrative skills. Able to prioritize varied projects. Dependable, flexible and tactful.

INTAKE COORDINATOR, PT

Location: Brevard County Responsible for the processing of all incoming patient referrals. Position works closely with the Nursing Department, Admissions Registered Nurse, and the Admissions Manager to ensure timely processing of all referrals. Position serves as a resource for questions/issues from the health care community and HOSF staff. Part-time. Requirements: High School Diploma; College Degree preferred. Excellent oral and written communication, clinical and administrative skills. Able to work collaboratively in a positive fashion with other health care professionals. Able to prioritize varied projects. Dependable, flexible and tactful.

SOCIAL WORKER, FT

Location: North Brevard County Responsible for identifying the psychosocial needs, goals and interventions of patients/families. In cooperation with the Interdisciplinary Team, a range of services may be provided including, but not limited to: assessment, counseling, advocacy, education, resource and referral services, and discharge planning.
Requirements: Masters Degree in Social Work from CSWE accredited school required. Education may be in a related field, such as counseling, psychology, sociology, gerontology. Position requires ability to work flexible hours. Experience in counseling and/or work with aging. Must be able to work independently and in an interdisciplinary environment.

BEREAVEMENT COORDINATOR, FT

Location: North Brevard County Responsible for implementing, managing and revising full range of grief support services to families and caregivers of hospice patients and the community as needed.
Requirements: Bachelors or Masters Degree in Social Work or related field. Ability to work flexible hours. Experience in counseling and/or work with aging. Must be able to work independently and in an interdisciplinary environment. Bereavement training.

TRIAGE RN, PT

Location: Brevard County RN is responsible for fielding and documenting telephone calls from patients and families. The work schedule for this position is every other weekend 7am – 5:30pm.
Requirements: Associate in Nursing required, BSN preferred. Two to three years of experience in home health, oncology, acute care or public health preferred. Must read, write and speak English; bilingual preferred.

BILLING SPECIALIST, FT

Location: Titusville, FL
Billing Specialist is responsible for all billing and collection activities on all patient accounts. Develop and input identified entries in the General Ledger on a monthly basis. Assist with completion of annual cost report.
Requirements: Bachelor degree in Finance/Accounting preferred with two (2) years progressive experience with one (1) year in a health care setting. Detailed knowledge of accounting and financial principles. Direct hands-on experience in billing and collection of Medicare/Medicaid accounts and third party payers. Analyze general ledger accounts and prepare monthly journal entries. Detailed knowledge of financial software programs, including electronic health record. Must be able to perform Accounts Payable and Payroll functions.

COMMUNITY ACCESS LIAISON, FT

Location: South Brevard County, FL
Position is responsible for identifying, qualifying and creating new business relationships. CAL required to meet routinely with personnel in physician’s offices, skilled nursing facilities, hospitals, assisted living facilities, churches, businesses and service organizations. Primary purpose is to create relationships that generate appropriate referrals.
Requirements: Bachelor degree in Communication/Public Relations, Marketing, Education or a medical related field. Two to three years experience in sales; work in either pharmaceutical or medical devices sales preferred. Must possess excellent interpersonal and verbal/written communication skills, excellent customer service and problem-solving ability and ability to work independently.
