CLINICAL NUTRITION COUNSELOR

Under the general direction of the Director of Clinical Services, the Clinical Nutrition Counselor is responsible for providing a complete assessment and recommendation of the nutrition Plan of Care for patients referred to him/her from the RN Case Manager. The Clinical Nutrition Counselor will be available for consultation on referred individual patients and for consultation to the RN Case Manager for nutrition education. The Clinical Nutrition Counselor will also have oversight of dietary services of the Care Center as a consultant dietitian. Position is per diem.

BEREAVEMENT COORDINATOR, MSW

Location: North and South Brevard County
The Bereavement Coordinator is responsible for implementing, managing, supervising and coordinating the full range of comprehensive grief support services to families and caregivers of hospice patients and to the community. The Bereavement Coordinator provides bereavement support via telephone, mail, and in person through individual grief support, memorial services, grief workshops, family assessment and conference.
Full-time is Monday through Friday and on call.
Requirements: Masters Degree in Social Work, Counseling or related field. Ability to work flexible hours. Experience in counseling and/or work with aging. Must be able to work independently and in an interdisciplinary environment. Bereavement training preferred.

RN CASE MANAGER, FT

Location: Float Position in Brevard County
RNs are responsible for accurate assessment of patient’s status, problems and needs. Coordinates hospice care for the patients and their family.
Float position is full-time and covers wherever needed on all shifts.
Requirements: Associate in Nursing required, BSN preferred. Two to three years of experience in home health, oncology, acute care or public health preferred. Must read, write and speak English; bilingual a plus.

LPN – CARE CENTER, FT

Location: Titusville
Provide direct patient care at the bedside while patient on continuous care level of care. Observe and inform on changes in patient’s condition and their response to interventions.
One full-time position open to work day shift with rotating weekends.
Requirements: Valid Florida LPN or RN license and valid Florida driver’s license. Minimum three years clinical experience, preferably in hospice, home health, oncology, acute care or public health. Bilingual a plus.

LPN – CRISIS CARE, FT

Location: Brevard County
Provide direct patient care at the bedside while patient on continuous care level of care. Observe and inform on changes in patient’s condition and their response to interventions.
One full-time positions open to work night shift with rotating weekends.  Two full-time position is open to work day shift with rotating weekends
Requirements: Valid Florida LPN or RN license and valid Florida driver’s license. Minimum three years clinical experience, preferably in hospice, home health, oncology, acute care or public health. Bilingual a plus.

CNA/HOSPICE AIDE, FT

Location: Brevard County
Provide personal care services and emotional support to hospice patients. Personal care services include, but are not limited to, personal hygiene, assistance with activities of daily living, transfers, and at times, light housekeeping.
Open position to work weekends covering the entire county.
Requirements: Valid Florida Certified Nursing Assistant. Two to three years experience as Certified Nursing Assistant or Home Health Aide. Reliable transportation needed.
