RN CASE MANAGER, FT

Location: North and South Brevard County
RNs are responsible for accurate assessment of patient’s status, problems and needs. Coordinates hospice care for the patients and their family.
Full-time is Monday through Friday and on call.
Requirements: Associate in Nursing required, BSN preferred. Two to three years of experience in home health, oncology, acute care or public health preferred. Must read, write and speak English; bilingual a plus.

RN CASE MANAGER, FT

Location: Float Position
RNs are responsible for accurate assessment of patient’s status, problems and needs. Coordinates hospice care for the patients and their family.
Float position is full-time and covers wherever needed on all shifts.
Requirements: Associate in Nursing required, BSN preferred. Two to three years of experience in home health, oncology, acute care or public health preferred. Must read, write and speak English; bilingual a plus.

LPN – CRISIS CARE, FT

Location: Brevard County
Provide direct patient care at the bedside while patient on continuous care level of care. Observe and inform on changes in patient’s condition and their response to interventions.
Two full-time positions open to work night shift with rotating weekends. One full-time position is open to work day shift with rotating weekends
Requirements: Valid Florida LPN or RN license and valid Florida driver’s license. Minimum three years clinical experience, preferably in hospice, home health, oncology, acute care or public health. Bilingual a plus.

CNA/HOSPICE AIDE, FT

Location: Brevard County
Provide personal care services and emotional support to hospice patients. Personal care services include, but are not limited to, personal hygiene, assistance with activities of daily living, transfers, and at times, light housekeeping.
Open position to work weekends covering the entire county.
Requirements: Valid Florida Certified Nursing Assistant. Two to three years experience as Certified Nursing Assistant or Home Health Aide. Reliable transportation needed.

PATHWAYS PROGRAM ASSISTANT, FT

Location: Titusville, FL
Position is responsible for providing administrative and telephone-based support to the program and team of providers (physicians and ARNPs).  Support includes processing incoming patient referrals and orders, managing the insurance verification and authorization process, scheduling, and providing billing support.  This position is responsible for establishing and maintaining positive relationships with customers and referral sources and responding to customer requests and concerns.  Position serves as a resource for questions and issues from the health care community, Pathways and HOSF staff.
Requirements: Associates degree or higher preferred.  Basic medical terminology required.  Medical office experience preferred that includes scheduling, billing and coding.  Excellent oral, written and electronic communication and administrative skills.  Able to work collaboratively in a positive fashion with other health care professionals.  Able to demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Requirements: Masters Degree in Social Work from CSWE accredited school required. Position requires ability to work flexible hours. Experience in counseling and/or work with aging. Must be able to work independently and in an interdisciplinary environment.
