RN CASE MANAGER, FT

Location: North and South Brevard County and Float Position
RNs are responsible for accurate assessment of patient’s status, problems and needs. Coordinates hospice care for the patients and their family.  Full-time is Monday through Friday and on call.
Float position is full-time and covers wherever needed on all shifts.
Requirements: Associate in Nursing required, BSN preferred. Two to three years of experience in home health, oncology, acute care or public health preferred. Must read, write and speak English; bilingual a plus.

ADMISSIONS RN, PER DIEM

Location: Brevard County
The Per Diem Admission Nurse (PDARN) is a Registered Nurse who is responsible for determining the eligibility and preparation of patient/family needs and for establishing the initial plan of care. This PDARN is also responsible for receiving and processing referrals and admissions from all settings (hospitals, skilled nursing facilities, assisted living facilities, and homes). This position works closely with the Admissions Team, Intake Coordinator, Vice President-Business Development, and clinical staff to ensure timely processing of all referrals.  All care coordinated by the PDARN is in conformance with federal/state regulations.
Requirements: Associate degree in nursing or a diploma certificate from an approved nursing program. Bachelor’s degree preferred.   Certification in Hospice and Palliative Nursing preferred. Must read, write and speak English; bilingual a plus. Three years experience as an RN, including a minimum of one year experience in a hospital setting. At least six months experience as an RN in a hospice preferred. Specific knowledge of provision of pain and symptom management therapies and interventions. Specific experience in providing care to hospice/palliative care patients in an inpatient setting. Knowledge of JCAHO standards for acute care and hospice desired. Excellent communication, clinical and administrative skills. Ability to prioritize varied projects. Able to work collaboratively in a positive fashion with other health care professionals. Must possess advanced critical thinking skills. Dependable, flexible, and tactful.

WEEKEND VISIT/TRIAGE RN, FT

Location: Brevard County
RN is responsible for fielding and documenting telephone calls from patients and families. Two positions are open – one work schedule for this full-time position is Friday through Sunday day shift and the other is Friday through Sunday night shift.
Requirements: Associate in Nursing required, BSN preferred. Two to three years of experience in home health, oncology, acute care or public health preferred.  Must read, write and speak English; bilingual preferred.

LPN – CRISIS CARE, FT

Location: Brevard County
Provide direct patient care at the bedside while patient on continuous care level of care. Observe and inform on changes in patient’s condition and their response to interventions. Four positions open – one day shift with rotating weekends and three night shift with rotating weekends.
Requirements: Valid Florida LPN or RN license and valid Florida driver’s license. Minimum three years clinical experience, preferably in hospice, home health, oncology, acute care or public health. Bilingual a plus.

CNA/HOSPICE AIDE, FT

Location: Central and South Brevard County and Float Position
Provide personal care services and emotional support to hospice patients. Personal care services include, but are not limited to, personal hygiene, assistance with activities of daily living, transfers, and at times, light housekeeping.
Float position covers wherever needed Monday through Friday and occasional weekend.
Requirements: Valid Florida Certified Nursing Assistant. Two to three years experience as Certified Nursing Assistant or Home Health Aide. Reliable transportation needed.

CNA/HOSPICE AIDE – CRISIS CARE, FT

Location: Brevard County
Provide personal care services and emotional support to hospice patients. Personal care services include, but are not limited to, personal hygiene, assistance with activities of daily living, transfers, and at times, light housekeeping. Schedule for position is Monday through Friday with rotating weekends.
Requirements: Valid Florida Certified Nursing Assistant. Two to three years experience as Certified Nursing Assistant or Home Health Aide. Reliable transportation needed.

SOCIAL WORKER, FT

Location: South Brevard County
Responsible for identifying the psychosocial needs, goals and interventions of patients/families. In cooperation with the Interdisciplinary Team, a range of services may be provided including, but not limited to: assessment, counseling, advocacy, education, resource and referral services, and discharge planning.
Requirements: Masters Degree in Social Work from CSWE accredited school required. Education may be in a related field, such as counseling, psychology, sociology, gerontology. Position requires ability to work flexible hours. Experience in counseling and/or work with aging. Must be able to work independently and in an interdisciplinary environment.

VOLUNTEER ADMINISTRATIVE ASSISTANT, FT

Location: Brevard County
The Administrative Assistant performs administrative, secretarial and clerical duties in support of the administrative functions requiring extensive knowledge of organization, policies, and procedures. Position performs tasks related to the Volunteer Department and general support of the Volunteer Coordinator.
Requirements: High School diploma or GED required. AA/AS or BA/BS in personnel management preferred. Additional training in medical terminology and medical coding helpful. Requires two years of related office/administrative assistance experience sufficient to acquire skills in office organization, documentation and communications. Word processing, spreadsheet preparation and database experience required. Excellent English oral and written communication skills.

ACCOUNTS RECEIVABLE SUPERVISOR, FT

Location: Titusville, FL
Under the general direction of the Finance Manager, the Accounts Receivable Supervisor will be responsible for ensuring compliance with state and federal laws, policies, and regulations.
Requirements: Associates degree in Business, Finance or related field. BA in Accounting or related field preferred. Detailed knowledge of how to process accounts receivable and all required documentation for internal and external (governmental) use. Complete mastery of Excel, Word and at least one accounting software package, preferably Sage. Prior Hospice/Healthcare experience preferred. Minimum of three years of prior supervisory experience; Accounts Receivable experience in Hospice/Healthcare Preferred; Experience with Medicare billing preferred; Detailed knowledge of processing accounts receivable, automatic computerized process for electronic billing submission, computerized payroll and accounts payable for a small to medium size firm. Medical terminology a plus.

QUALITY IMPROVEMENT COORDINATOR, FT

Location: Titusville, FL
Under the general direction of the Compliance Officer, the Quality Improvement Coordinator is responsible for the development, coordination and implementation of the Quality Assessment and Performance Improvement Program. Scope of responsibility includes comprehensive quality improvement activities for the organization. The Coordinator has accountabilities for the monitoring, implementation and evaluation of process and programs to meet regulatory, accreditation and standards of practice.
Requirements: Registered Nurse, required. Bachelor’s degree in Nursing preferred.  Five years experience in a healthcare organization and a minimum of three years in a hospice, to include demonstrated clinical leadership. Specific healthcare work experience in quality improvement. Proficient in utilizing electronic medical records in decision making. Possess strong analytical skills. Knowledge of current hospice regulations.
