Care for Patients and Care for Families

We serve patients and their families at home, in nursing homes, in hospitals and at our care center. We offer comprehensive care and support services, helping patients to manage their symptoms and giving families and caregivers the resources and assistance they need.

Caregiver Education Program

Hospice of St. Francis offers a free national training program to help all family caregivers – past, present or future.

Edward M. Poe Hospice Care Center

The Care Center is a 12-suite full service hospice care facility conveniently located off of I-95 in Titusville.

Grief and Support Groups

Hospice of St. Francis supports and helps family members who are grieving the loss of a loved one whether or not he or she was our patient. This is done through private sessions in your home or in one of the Hospice of St. Francis offices or in one of the adult grief support groups.

North Star for Children Program

Children grieve differently. Hospice of St. Francis’s North Star Program helps children, teens and families heal.

St. Francis Pathways to Healthcare

We are a local, experienced team of Healthcare professionals dedicated to the compassion for and comfort of people facing serious illness.

Volunteer Services

Volunteers are a vital member of the Hospice Care Team. They provide companionship, support and assistance to patients residing at home, in assisted living facilities, nursing homes, hospitals and our care center.
